When processing a Private Health Insurance claim, you are required to add your Private Health Insurance card details. 

If you're with Medibank, this includes the card issue date. For most Medibank members, this can be found on the front of your card, at the bottom right-hand corner.

Some newer Medibank cards don't have the card issue date listed on them.

If your card does not have the card issue date, your issue date will be 1 or 2 days after your policy start date. You should give both dates a try. 

Your policy start date is emailed to you when you sign up to Medibank, or alternatively you can contact Medibank who will be able to advise your policy state date to you.

We understand that this can be frustrating, however, it is still a required field for us when processing Medibank claims.
