If you already have an approved HBF Member Plus Dental agreement within Medipass and you are looking to amend it as staff are joining or leaving your team, follow these steps.

First you will need to add any staff members that are joining the team or remove the staff members leaving the team. 

Once you are done adding staff, you will need to submit a "HBF Member Plus Dental - Change Practitioner" form. This can be found within 'Documents'. 

Scroll down to the bottom and hit 'Apply', then select the practice that you have added more staff to, fill in any missing details, review and submit it. Your amended HBF agreement with your new staff will take affect after it is approved.
